Regular Physical Activity is fun and Healthy, and increasingly more people are starting to become more active every day. Being more active is very save for most people. However, some people should check with their doctor before the start becoming more physically active. If you are planning to become more physically active then you are now, start by answering the questions below. If you are between the ages 15-69 these questions will determine if you need to check with a doctor before you start. If you are older than 69, and/or unaccustomed to being very active, check with your doctor anyway.

I hereby grant authority to Siemerworks LTD (SW). and/or affiliates to provide me with personal training services. The trainers are not health practitioners and cannot be expected to diagnose and or treat individual health problems. Health concerns should be addressed by me, to my own physician, and I agree to do so. Also, I am aware that any health concerns I may discuss with the trainer(s) will be done so in full confidentiality unless I give written permission to the trainer to discuss unless I am a minor. I am in good health and/or cleared by a licensed health practitioner to be in good health, and fully able to participate in a fitness program. If necessary, I also give the trainer to discus health concerns with my Physician or medical personnel if necessary. I understand that the trainer may consult with another trainer in my programming in which I give full authority to do so. I hereby acknowledge that in using any equipment, program, and/or facilities with SW Trainers, staff, or contractors, I do so entirely at my own risk. Therefore, I release all staff, Owners, trainers, contractors, and affiliated facilities from any liability for injury or loss except in case of willful misconduct and/or negligence. This represents the the party's final expression of intentions as it relates to the services to be received.
